Activities

During recovery and rehabilitation, it’s important to keep your body and mind active. Between the activities organized by our team and the activities provided by our partnered organizations and the community of the Greater Cleveland, Ohio area, there’s always something fun going on!

We have a daily schedule of activities like trivia, musical performances, movies, and games being held in our facility, as well as several outings and gathering events that occur throughout the month, so no matter what your tastes are, there’s always an activity for you.

Room Options

Mentor Hills Post Acute offers semi-private and private rooms. Our rooms offer both safety and privacy with the comforts of home. We encourage residents to bring personal items such as family photos, blankets, and decorative items to feel as comfortable as possible.

Dietary Program

Our registered dietitian follows physician orders to create meals that meet the specific dietary and therapeutic needs of our patients. Our team will also conduct regular nutritional assessments to ensure meals meet residents’ changing needs. These carefully crafted meals support a variety of residents’ needs including:

There’s nothing more important to us here at Mentor Hills Post Acute than our residents. We know all too well that even during the recovery process, life doesn’t slow down. Our social service team assists both you and your family during the transitional periods you will experience. Our social workers provide essential information, manage requests and concerns, and help in your care planning and discharge planning.

Resident Advocacy 

Our social services team is committed to protecting each resident’s rights, dignity, and personal choices. We advocate on behalf of residents to ensure their voices are heard, their preferences are honored, and their concerns are addressed promptly. By collaborating with care teams and families, we help promote a respectful, person-centered environment where residents feel empowered and supported.

Legal Assistance 

We help residents and families navigate important legal matters related to care and planning. This may include guidance on advance directives, power of attorney, and access to community legal resources. Our goal is to ensure residents are informed, protected, and supported in making decisions about their care and future with confidence. 

Mental Health Services 

Emotional well-being is an essential part of overall health. Our social workers provide support, counseling, and referrals to address depression, anxiety, adjustment challenges, and other mental health needs. We work closely with residents to promote coping, engagement, and a positive quality of life during their stay. 

Discharge Planning & Transitions 

We assist residents and families in preparing for a safe and smooth transition to the next level of care, whether returning home or moving to another setting. From coordinating services and equipment to connecting with Mentor’s community resources, our team ensures each resident leaves with a clear, personalized plan that supports continued health and independence.
