Our Mission

At Mentor Hills Post Acute, our mission is to be a skilled nursing and rehabilitation center that provides comfort, support and care in a home-like setting. During your stay here, you can expect the highest level of care and rehabilitation to aid in your recovery while participating in engaging activities, view the beautiful courtyards in the fresh air, and enjoy tasty meals with friends. While we were formerly known as Legacy of Mentor, and then Heartland of Mentor, we updated our name to Mentor Hills Post Acute to better reflect our dedication to our mission to provide the people of Mentor, OH with high-quality post acute care.
