My uncle is in there and the nurse are leaving him wet.. I just want him better take care of and not neglect..

This place is a nursing home, and I wouldn’t put anyone I cared about in there.

The Physicians dont check the patients there. If bloodwork shows a problem they send in a nurse practitioner. Go somewhere else. Definitely a NO GO! They dehydrated my family member so severely it could’ve killed them. Stay away!!

I will not be recamendig any to that facility because the care they give there pacient sucks they staff acts like they don’t care about there clients.I have been there to see family member a few times and I did not like what I saw.There fore I am trying to her transferd out of that facility .

My father is currently a resident at Mentor Hills Post Acute. I visited from out of town last weekend and was so happy to see what a lovely facility it is. I walked in to see one of the nurses feeding him with such gentleness and care, it gave me peace knowing he is well taken care off. The facility is spotless and located in a wooded setting and is just beautiful. The staff has been really kind to my mom and the entire family wishes to thank everyone for the great care my dad receives. We all appreciate you so much! EDIT: My father passed today and I wanted to give a special thank you to nurse Kim W. The care and compassion she showed in his final days and hours was more than I could have ever imagined. Thank you from the bottom of my heart.

My father in law was at Mentor Hills Post Acute Care. The staff was extremely attentive and did whatever they could to accommodate him and our family. I am grateful they were there during such a difficult time.

After comparing this facility to another facility I was at, I have found that mentor hills post acute is far superior in the care that you receive. The people are kind and treat me nice. There are activities to keep you busy all the time if you wish to be. Movies on Friday night I think that’s great, excellent church services, bingo and other games. Highly recommend if you are looking for a skilled nursing facility.

Have had nothing but problems with my grandfather and getting his bedtime medicine and getting help from the overnight staff. Extremely rude. **update** Sarah has been very helpful with getting my grandfather situated and the transfer home. However some of the staff was still very slow to respond in taking 45 minutes to get to my grandfather to the bathroom.

This place is a joke. Staff overworked, not trained properly, not friendly with residents or families. I would like to see the owner’s to put their loved ones here. Picture are not even of this location. Now we can not get the code for the front door to be able to come and go to visit loved ones.

My grandma has been there for a month. The staff is friendly, helpful and has gone way above to help her in any way she needs. We have been very happy with grandma’s stay. Would definitely recommend.

They should keep the same staff on the memory care unit, but they don’t. They are understaffed and you have to hunt nurses and aides. The unit is very small and gives a not homey vibe.

I recently contacted facility with a family friend to go over some financial details with the business office manager Melissa. She was very knowledgeable and helpful in assisting the family with their questions so we stopped in to tour the place. Facility was clean and everyone was friendly, potential resident will be admitting soon. Thank you so much for your hospitality Melissa!

They took great care of mom at EOL. the staff is wonderful. Lots of one on one with mom. I slept every night confident that she was in good hands and taken care of.

They took amazing care of my aunt, the staff were accommodating to every question and need, they were professional and just treated us like family which meant the world to us. I can’t thank them enough!
