I have been to Mentor Hills rehab multiple times. Everyone is very sweet understanding and helpful. Their PT‘s and OT’s are amazing. I would recommend this to anybody to come to or put their loved one in. They also have a lot of nice activities, Church groups, and entertainment that come in.

everything has been great very caring and loving staff. only complaint is the food is not great. but I highly recommend the facility for others looking for rehabilitation.

The activities here is coordinated with precision and excellence. From daily preparation of breakfast, lunch and dinner, and the therapy section is coordinated in excellence. I recommend to this to both family and friends and all those that are seeking a rehab center.

Zero star for Memory Unit. All families need to speak up if you see staff on their cell phones. Phone policy should be posted on bulletin boards and monthly letter that is mailed out. The charting and communication at this facility is horrible. Our family is being labeled as unrealistic because we expect the care plan to be followed. My father is a stand my assist because he can barely stand. He is expected to use the bathroom by himself. He keeps falling in the bathroom. They don’t check his pullup/briefs. I count pullup/briefs before leave, he has 7, come back 24 hours later and there are 6. Department of Health needs inspected also. The memory unit should have never been approved in the first place back when it was Heartland of Mentor. The people that run these places and keep changing names to hide stuff should be held responsible. Disgusting world we live in. I don’t care what side you are on; the Government and the Rich are the problem!

One of the worst facilities I’ve ever seen. Let’s start off with the smell & how filthy that place is. I was under the impression no one cares about the patients based off attitudes & no follow ups on complaints that we had. As far as management the Head of Nurses definitely just there for a pay check walks by hanging her down doesn’t show any empathy. The administrator is young & clueless and looks lost when talking speaking to her. Do not put your loved one there that place is under staffed and a disappointment!

I went to visit a friend and bring him Communion this past weekend. I was appalled at the uncleanliness of the room. and stickiness of the floor. I took 5 minutes out of my visit to clean up the room , pick up his items that had fallen on the ground, and reorganize so he had acces to his personal items. There were no towels or wash cloths in his room or bathroom. I find it hard to believe that the workers would not provide this service at least a few times a day in each of the patients’rooms. When I entered the building, I saw many employees standing around the nursing station. One of them was sitting with their head on the table sleeping. Granted I may have caught them after performing difficult rounds, but to see three employees around doing nothing when my friend’s room was in disarray is very disheartening. I say this because these individuals deserve dignity and respect just like all human beings do that are not in a facility recovering from an illness.

The staff is very kind, on top of what you need, dont need to wait a long time. After coming here i feel safer than ive been in years. Admissions was made really easy.

Do Not Go ! No Communication re his care. Recieved majority of info from my husband and his daughter. He was referred to two other specialist while there and never saw them. We were told on discharge he was set up with Capital Home Health Care. Discharged on 8/20/2026. I finally called the home health care agency on 8/26/2026 and was told he was denied on on 8/20/2026 due to insurance. They notified Mentor Hills of this but no one from Mentor Hills notified us. My husband was discharged on 8/21/2026. My error.

Nurses are great!! My husband was there for 5 months and his Nurses were johnny on the spot!! PT department is great as well!! Only complaint from husband is the food..but he has to go back unfortunately..but we chose this place because the Nurses do care

Aids don’t actually help! They stand around and play with their phones. Sarah the Administrator is amazing! She takes care of all the Residence and is involved with all the staff. Thank you for the extra care!! Laundry services continue to loose my clothes and do not deliver in a timely manner. I have been wearing the same clothes for about a week!

Angie is amazing the aide. Lito is a great nurse. Highly recommend. Overall the nurses are nice and the staff treats us good.

I like it here because good therapy, lots of activities, and the nurses care about me. Everyone is treated like family here.

The Mentor Hills team genuinely cares for its residents & staff. I can speak from firsthand experience. From Therapy to Wound care, Mental Health, Nursing & NP services – this facility is a great choice for rehab or long term care.

My uncle is in there and the nurse are leaving him wet.. I just want him better take care of and not neglect..

This place is a nursing home, and I wouldn’t put anyone I cared about in there.

The Physicians dont check the patients there. If bloodwork shows a problem they send in a nurse practitioner. Go somewhere else. Definitely a NO GO! They dehydrated my family member so severely it could’ve killed them. Stay away!!

I will not be recamendig any to that facility because the care they give there pacient sucks they staff acts like they don’t care about there clients.I have been there to see family member a few times and I did not like what I saw.There fore I am trying to her transferd out of that facility .
