Do Not Go ! No Communication re his care. Recieved majority of info from my husband and his daughter. He was referred to two other specialist while there and never saw them. We were told on discharge he was set up with Capital Home Health Care. Discharged on 8/20/2026. I finally called the home health care agency on 8/26/2026 and was told he was denied on on 8/20/2026 due to insurance. They notified Mentor Hills of this but no one from Mentor Hills notified us. My husband was discharged on 8/21/2026. My error.

Nurses are great!! My husband was there for 5 months and his Nurses were johnny on the spot!! PT department is great as well!! Only complaint from husband is the food..but he has to go back unfortunately..but we chose this place because the Nurses do care

Aids don’t actually help! They stand around and play with their phones. Sarah the Administrator is amazing! She takes care of all the Residence and is involved with all the staff. Thank you for the extra care!! Laundry services continue to loose my clothes and do not deliver in a timely manner. I have been wearing the same clothes for about a week!

Angie is amazing the aide. Lito is a great nurse. Highly recommend. Overall the nurses are nice and the staff treats us good.

I like it here because good therapy, lots of activities, and the nurses care about me. Everyone is treated like family here.

The Mentor Hills team genuinely cares for its residents & staff. I can speak from firsthand experience. From Therapy to Wound care, Mental Health, Nursing & NP services – this facility is a great choice for rehab or long term care.

My uncle is in there and the nurse are leaving him wet.. I just want him better take care of and not neglect..

This place is a nursing home, and I wouldn’t put anyone I cared about in there.

The Physicians dont check the patients there. If bloodwork shows a problem they send in a nurse practitioner. Go somewhere else. Definitely a NO GO! They dehydrated my family member so severely it could’ve killed them. Stay away!!

I will not be recamendig any to that facility because the care they give there pacient sucks they staff acts like they don’t care about there clients.I have been there to see family member a few times and I did not like what I saw.There fore I am trying to her transferd out of that facility .

My father is currently a resident at Mentor Hills Post Acute. I visited from out of town last weekend and was so happy to see what a lovely facility it is. I walked in to see one of the nurses feeding him with such gentleness and care, it gave me peace knowing he is well taken care off. The facility is spotless and located in a wooded setting and is just beautiful. The staff has been really kind to my mom and the entire family wishes to thank everyone for the great care my dad receives. We all appreciate you so much! EDIT: My father passed today and I wanted to give a special thank you to nurse Kim W. The care and compassion she showed in his final days and hours was more than I could have ever imagined. Thank you from the bottom of my heart.

My father in law was at Mentor Hills Post Acute Care. The staff was extremely attentive and did whatever they could to accommodate him and our family. I am grateful they were there during such a difficult time.

After comparing this facility to another facility I was at, I have found that mentor hills post acute is far superior in the care that you receive. The people are kind and treat me nice. There are activities to keep you busy all the time if you wish to be. Movies on Friday night I think that’s great, excellent church services, bingo and other games. Highly recommend if you are looking for a skilled nursing facility.

Have had nothing but problems with my grandfather and getting his bedtime medicine and getting help from the overnight staff. Extremely rude. **update** Sarah has been very helpful with getting my grandfather situated and the transfer home. However some of the staff was still very slow to respond in taking 45 minutes to get to my grandfather to the bathroom.
