Through the years, we’ve been known by a few different names. We’ve served the Mentor community formerly as Legacy of Mentor, then as Heartland of Mentor, and now as Mentor Hills Post Acute.

This new name, Mentor Hills Post Acute, is more than just a rebrand. It represents a renewed promise to the individuals and families who trust us during some of life’s most challenging moments. Our evolution into Mentor Hills Post Acute signals a meaningful new chapter rooted in growth, leadership, and elevated care.

While our name has changed over time, our mission has remained constant: to provide compassionate, high-quality post acute care in a setting that feels supportive, professional, and personal. Today, that mission is stronger than ever.
