Who We Are
Through the years, we’ve been known by a few different names. We’ve served the Mentor community as Legacy of Mentor, then as Heartland of Mentor, and now as Mentor Hills Post Acute. Through all these name changes, one thing has remained consistent; our passion for high-quality post acute care. So, whether you’ve known us as Legacy of Mentor, Heartland of Mentor, or Mentor Hills Post Acute, we’re still dedicated to delivering the best post acute and rehabilitation care possible.

We are proud to announce that we’ve been recognized by the Centers for Medicaid and Medicare Services (CMS) as a Five-Star Quality Measures Rated facility. Nursing homes are rated from 1-5 stars based on health inspections, staffing, and quality measures. According to CMS, a rating of five stars means that quality is “much above average.”
